COST OF SERVICES FOR 2009

New Patient First Visit & Two Health Assessment Tests $418 (up to 1 hour)
New Patient Follow Up Visit $154—$302 (30—60 min)
Established Patient Follow Up Visit $154—$302 (30—60 min)

*All prices are approximate. Extra fees will be applied lab tests, blood work, vitamin or supplement purchases and extra time spent with the doctor.

INSURANCE COMPANIES

We do not have direct relationships with any insurance companies.  However, we will provide you with a preprinted claim form to submit to your insurance company.  The form contains all the necessary codes and diagnosis pertaining to the office visit.  The patient only needs to make a copy of his or her insurance card and submit both documents to his or her insurance company.  It is your responsibility to pay for services when rendered and then seek reimbursement from your insurance company. 

MEDICARE PATIENTS

Preventive Medicine is an opt—out—provider of Medicare.

What does opting—out of Medicare mean to me as a patient?

When a healthcare provider opts out of Medicare the patient gives up all Medicare coverage and reimbursement for services furnished by the opt-out physician.  The Medicare patient is not allowed to submit or ask the physician to submit any claim to Medicare for services provided by the opt—out physician.  The patient is also no longer charged Medicare limits; therefore, the patient is charged the same rate as a non-Medicare patient.

Will claim(s) go to Medicare for procedures ordered by Dr. Rathod?

Yes, if the doctor orders a patient to have any type of routine testing or lab procedure to be done at another site, office or hospital, the claim(s) will go directly to Medicare if that entity is a Medicare provider.

How can I most cost effectively use Dr. Rathod?

You have the option of using Dr. Rathod as your primary care physician or as a wellness consultant.  As a wellness consultant, you can use the doctor for the purpose of IV’s, nutritional, holistic, and preventive therapies. 

One option to help cut costs is to have a primary care doctor, who accepts Medicare, perform annual physicals.  By doing this you are able to have your yearly exam submitted to Medicare and use Dr. Rathod more as a holistic wellness consultant.  You can also use her for other issues, questions and/or problems you may have.
